CHeckBox instead of button to purchase music downloads

Does any one know how to make a checkbox instead of a ?button to purchase song?
here is an example :

this site actually uses zen cart but same idea

I want to do it like this using listing different songs line by line and using flash to play demo

1  how to make the product purchase with a checkbox?

2 how do i keep the songs close together (line by line)?

3 Which folder would I Locate the swf file in for flash in a given page?

4 Which folder would I Locate the flash for the same given page?

5 also if I were to call a javascript (for something unrelated) where should I locate the javascript or doesn't it matter, store root etc?
